Accident and Rescue Summary
Sun and warm weather seemingly triggered an avalanche above me, and I was swept 150 yards down the slope, was buried and broke my arm. My dog was carried in the avalanche too.
Here is my best determination on location of impact:

7,555 ft (my elevation at impact, probably originated around, 8,000)
A northern slide then turning left at the gully and going west. My stopping position 280 yards later (per Google Earth):

I don't know if it was a loose or slab avalanche. Because my arm was broken and had a chest contusion from the impact, I needed to get off the mountain, I regretfully did not take photos. I'll attach those I do have.
I was snowshoeing alone with my mini-aussie dog. The snow I was walking on was holding. It was windy and blowing snow off the ridgeline above me where I assume the avalanche originated. I was hearing booming noises most of the ascent from what I believed was home construction in the valley below. Just before impact, I heard thunder and a boom. I was looking up to the sky thinking it was weather when I was blind sided by the avalanche. I did not see it coming.
Slide Depth: 4-5' (hit me in the chest and I'm 6'2")

I came to a stop mostly buried. It took approx. 20 minutes to become unstuck. I'm unaware how the dog was impacted, but she lived.

They were trying to call me to get a location, but again I was not using my phone and I did not know it was ringing in my fanny pack.
A life flight landed in the church parking lot across the street to help locate me, but I had already self rescued to the snow line where I was met by rescuers and vehicles.
